Quality Assurance Coordinator
Primary responsibility is to ensure that all agency programs are in compliance with OMRDD and the Department of Health regulations and to conduct investigations. Bachelor’s Degree in related field required. With a minimum of 5 years experience working in the OMRDD or DOH field.
Residential Program Managers
Responsibilities include the overall operation of the residential
group home, including supervising and managing the Assistant
Program Managers and the Residential Support Counselors. Ensure
that the appropriate care is administered at all times for our
consumers, as well as perform the responsibilities of the
Assistant Residential Program Manager and the Residential Support
Counselors when necessary. Requirements: Bachelors
Degree plus 2 years of experience or High School Diploma with
4 years of relevant experience required; Master’s Degree
preferred; clean valid NYS driver’s license; prior supervisory
experience a plus.
Assistant Residential Program Managers
Responsibilities include assisting the Program Manager with supervising
Residential Support Counselors by assigning specific activities
to encourage each consumer to strive for autonomy; maintaining
weekly work schedules for all staff by ensuring assignments of
shifts based on availability and the particular requirements
of each consumer to ensure that appropriate care is administered
at all times; as well as perform the responsibilities of
a residential support counselor when necessary. Requirements: HS
Diploma with one year of experience in a residential setting
and a clean valid NYS driver’s license; flexible hours
with 1 weekend shift required.
Residential Support Counselors
Work directly with survivors of traumatic brain injury in a group
home setting. Responsibilities include supervising and
assisting the consumers in activities of daily living skills
such as personal care, social and community recreation, medication
and health care issues, encouraging consumers to be as independent
as possible; as well as transporting individuals to community
resources. Requirements: HS Diploma/GED; clean valid
NYS driver’s license. Full time/Part time/Per Diem – all
shifts available.
Independent Living Skills Trainers (ILST)
These trainers provide one-to-one training to consumers with TBI based on an assessment of needs. Trainers assess the skills and abilities of the consumer in managing their daily living activities including personal care, independent living skills, social and community recreation, medication and health care issues. Requirements: BA/BS plus 1 year of experience or HS Diploma and 2 years of experience providing functionally based assessments, developing a comprehensive treatment plan and teaching individuals with disabilities to be more functionally independent. Flexible hours; Clean NYS driver’s license with own vehicle required.
Intensive Behavioral Program Services (IBP)
Responsibilities include behavior observations, data collection, documentation and implementation of the behavior support plan. Provide behavior intervention to consumers and train all staff using functional analysis designed to decrease challenging behaviors due to TBI. Excellent clinical opportunity. Requirements: BA/BS with 1 year of related experience working with people with traumatic brain injuries, other disabilities or behavioral difficulties. Flexible hours including evenings; Clean NYS driver’s license with own vehicle required.
Community Integration Counseling (CIC)
Provide one-to-one counseling sessions to survivors of TBI and
their families. Assist the survivors to effectively manage
the emotional difficulties associated with adjusting to & living
in the community. Requirements: MA in Psychology, Social
Work, MSW or CRC with two years experience providing adjustment-related
counseling to individuals with traumatic brain injuries and their
families. Suffolk/Nassau locations. Flexible Schedules,
Part Time, Evenings, Weekends.
Service Coordinators
The primary responsibility of the Service Coordinator is to work
in partnership with the consumers to enable them to live and
participate in the community according to their personal goals,
preferences, and needs. Requirements: Masters in
Social Work or related degree; or Bachelor’s Degree plus
1 year of experience in Developmental Disabilities/Service Coordination
required. Clean NYS driver’s license with own vehicle
required.
At Home Residential Habilitation Counselors
Responsibilities include working with OMRDD/TBI survivors one-to-one in their homes or residential settings providing independent living skills assistance, escorting them into the community, and assisting them with completing their Activity of Daily Livings Skills (ADLS). Requirements: HS Diploma and prior experience working with people with disabilities required. Clean NYS driver’s license with own vehicle required.
